Occupational Therapist, Home Based Care, Southern Sydney This role is 100% home based, supporting people in aged care and people living with disability across Sydney's southern suburbs. You'll be joining a mobile allied health provider with an established, collaborative multidisciplinary team.
About the role
- 100% adult caseload, made up of aged care (Support at Home / CHSP) and NDIS disability support
- Home safety assessments, assistive technology prescription, home modifications and functional capacity reporting
- Appointments scheduled within roughly a 30 minute drive of your own home base
- Home based role, with occasional access to our clinic offices for clinic based work if needed
- Monthly in person team meetings held at one of our clinic locations
Key responsibilities
- Conduct comprehensive assessments and develop individualised intervention plans for adult clients
- Prescribe and review assistive technology, equipment and home modifications
- Prepare clear, well documented reports and recommendations
- Liaise with support coordinators, case managers, families and other allied health providers
- Contribute to a collaborative multidisciplinary team alongside Physiotherapists, Exercise Physiologists, Speech Pathologists and fellow OTs
The team
- You'll join a wider Occupational Therapy and Speech Pathology team of around 10 clinicians
- The broader clinical team includes around 12 Physiotherapists and Exercise Physiologists, supported by 3 administrative staff who manage scheduling and non-clinical logistics
- The full team meets fortnightly, alternating between in person and online sessions
Requirements
- Degree in Occupational Therapy with current AHPRA registration
- Comfortable working autonomously in clients' homes, with sound clinical reasoning and reporting skills
- Full time preferred, though 3 to 4 days a week can be considered for the right candidate
- Own car and driver's licence
What's on offer
- $80,000 to $85,000 base salary, depending on experience
- $1.01 per km car allowance, plus tolls and parking reimbursed (no car is provided, but running costs are fully covered)
