Position Responsibilities: · Perform comprehensive nursing assessments addressing physical, emotional, psychosocial, and spiritual needs. · Collaborate with the interdisciplinary team to develop, implement, and evaluate personalized care plans. · Provide skilled nursing interventions including wound care, medication management, IV therapy, symptom/pain control, and patient/family education. · Deliver emotional support and education to patients and caregivers based on individual needs. · Supervise LPNs/LVNs and home health aides while coordinating care activities and documentation. · Respond to urgent needs, participate in admissions/on-call, and adjust plans based on ongoing monitoring. · Ensure accurate, compliant clinical records and high-quality, compassionate care delivery. Requirements
- Prefer someone with Home Care/Home Health experience but not required.
- One (1) year experience as a RN.
- Must possess and maintain valid CPR certification while employed in a clinical role.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
