Job Description Position Purpose Supports facility providers, supervising physicians, and the Medical Director by preparing for rounds, coordinating provider-directed orders and diagnostic services, and communicating clinical information. Performs assigned clinical support duties within LPN scope, demonstrated competency, provider direction, and facility policy.

Essential Responsibilities

  • Prepare and organize the weekly provider-round resident list, records, laboratory results, reports, and notes for the Medical Director and other providers. Assist with provider-requested letters, forms, and documentation for authorized outside entities; scan or route outside reports into the EMR for provider review.
  • Assist providers with resident examinations and minor procedures as directed, including preparing the resident and equipment. Coordinate diagnostic testing and imaging requests; route reports to the appropriate provider or unit. Perform clinical tasks only when trained and authorized.
  • Coordinate resident preparation and information flow for scheduled provider rounds; accompany rounds when assigned. Route psychiatric, pharmacy, dietary, laboratory, and other clinical review notes to providers for consideration; do not independently change a treatment plan or treat a recommendation as an order.
  • When physically present in the building, the attending provider is responsible for entering orders; the Provider Nurse does not enter orders on the provider’s behalf. Only when the provider, supervising physician, or Medical Director is physically absent may the Provider Nurse enter verbal/telephone orders, as permitted by facility policy and with required read-back, documentation, and authentication. Clarify incomplete or conflicting orders. Coordinate new admission order needs and diagnostic requests as assigned and notify the care team.
  • Maintain assigned routine and annual laboratory schedules and coordinate diagnostic imaging requests, including calls, faxing, and delivery of copies as needed. Assist in tracking laboratory results from collection through final disposition, including preliminary and final cultures, in collaboration with phlebotomy and the Infection Preventionist (IP) nurse. Route results to the ordering or covering provider within policy timelines; confirm review and follow-up, document actions, and escalate critical results or delays per policy. Do not independently interpret results.
  • Assist with monitoring resident follow-through with ordered treatments. Identify and escalate missed appointments, unavailable results, and other barriers to care; document follow-up and resolution or escalation.
  • Gather assigned quality assurance (QA) monitoring data and communicate incomplete or overdue documentation and other overdue items to clinical and administrative leadership. Promptly communicate changes, questions, and follow-up needs to the provider, RN Unit Manager, House Supervisor RN, DON/ADON, or other appropriate team member and document communication in the designated record.
  • Perform other duties as assigned, consistent with Oklahoma LPN licensure, demonstrated competency, applicable law, and facility policy.

Minimum Qualifications Current, unencumbered Oklahoma LPN license; proficiency in English reading and writing sufficient to understand clinical instructions and policies and complete accurate documentation; basic EMR/computer skills; effective communication; and ability to maintain required training and competency.
