Actively Hiring Concierge Registered Nurses Shifts: 12 Hour DAY, NOC, and 2-4 hour short shifts Location(s): La Jolla, North San Diego County Hourly Pay Rate: $45-$80 based on experience About the Opportunity At Cheer Health Care, we view nursing as relationship-centered care rather than just checking off clinical boxes. We are currently welcoming a compassionate Registered Nurse (RN) to our home care team to deliver one-on-one, person-centered support for older adults in their own residences. In this role, you will work closely alongside an interdisciplinary care team to design, manage, and adapt individualized treatment plans while maintaining transparent, continuous communication with patients, families, and peer clinicians. Key Clinical Responsibilities:
• Intake & Ongoing Evaluations: Conduct comprehensive physical assessments and evaluate ongoing client requirements.

- Goal-Oriented Care Planning: Formulate personalized care strategies and establish achievable health milestones.
- Long-Term Condition Oversight: Deliver proactive chronic disease management to reduce hospital readmissions.
- Specialized Interventions: Administer targeted wound treatments, dressing changes, and post-operative recovery care as required.
- Empowerment & Guidance: Provide essential health education and coaching to clients and their primary caregivers.
- Preventative Support: Promote overall health maintenance and independent daily living.
- Current, unrestricted California registered nurse license
• 3 years of experience as a registered nurse
- 3 years of experience as a registered nurse
- Home Health experience preferred
- Proof of Negative TB result
- Physical Screening Documentation within the last year
- Active California Drivers license and car insurance
