Job Description
- The Patient Care Coordinator is a vital member of the healthcare team, responsible for overseeing the continuity and efficiency of patient care services.
- This role involves coordinating between various healthcare providers, managing patient schedules, and ensuring patients receive the necessary care in a timely and organized manner.
Key Responsibilities
- Care Coordination: Ensure seamless coordination of patient appointments, treatments, and follow-up visits across various departments.
- Patient Communication: Serve as the primary point of contact for patients, providing information about treatment plans and answering inquiries.
- Medical Records Management: Maintain accurate and up-to-date patient records, ensuring that all relevant health information is available to appropriate medical staff.

- Interdepartmental Liaison: Act as a liaison between different departments within the healthcare facility to facilitate efficient patient care.
- Patient Education: Educate patients and their families about health conditions and care plans, improving their ability to manage their health.
- Feedback Collection: Gather patient feedback to identify areas for improvement in service and care delivery.
Education Qualifications
- Education: Associate's or bachelor’s degree in Healthcare Administration, Nursing, or related field.
- Experience: Previous experience in a healthcare setting, particularly in patient coordination or administration.
- Skills: Strong organizational, communication, and interpersonal skills; proficiency in medical software is often required.
