Location: Home Health - Fort Worth Department: Administration Shift: First Shift (United States of America) Standard Weekly Hours: 40 Summary: The LVN, Case Manager Liaison will coordinate efficient and effective transition for patients from hospital setting to home environment while maximizing patient outcomes.
Education:
- The formal education from an accredited college or university in area of nursing. Education and job training as a Licensed Vocational Nurse with current State of Texas licensure (or license eligible) is required.
- Minimum of 3 years pediatric experience required; 2 years of home health and/or discharged facilitation/case management experience preferred.
- Knowledge and understanding of pediatric home care.
Certification/Licensure:
- Current State of Texas licensure (or license eligible) is required.
- Basic life support (BLS) certification or scheduled attendance of a CPR certification course.
