Requirements Assists in the planning, implementation, documentation and coordination of patient care services to Hospice patients and their families. Qualifications:
• Current NYS Registered Nurse license.
- Current NYS Registered Nurse license.
- Two (2) years experience in nursing which includes any combination of medical-surgical, oncological, palliative and community home health/hospice.
- At least one (1) year of community hospice or home health experience is preferred.
- Hospice experience preferred.
- Position requires ability to commute to multiple territories to ensure continuity of care.
- A valid driver’s license is required. (This requirement does not apply to positions assigned exclusively to Manhattan territory)
- A vehicle is required to commute to patients. (This requirement does not apply to positions assigned exclusively to Manhattan territory)
- Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.
- Baccalaureate Degree in Nursing from an accredited school is preferred.
Location : Manhattan Hours: 9am - 5pm Salary: $113,576 - $140,076 annually
