Physical Therapist (PT) - 1099 Always Compassionate Health
Schedule: Flexible Per Diem | Immediate Opportunities Location: Nassau County, Long Island, NY Employment Type: 1099 Independent Contractor
Always Compassionate Health is seeking compassionate, reliable, and dedicated Physical Therapists (PTs) to provide high-quality rehabilitation services to patients in their homes. Key Responsibilities
- Conduct physical therapy evaluations to assess mobility, strength, balance, and functional limitations.
- Develop and implement individualized treatment plans based on patients’ needs and goals.
- Provide therapeutic exercises, gait training, balance training, and mobility interventions.
- Monitor patient progress and adjust treatment plans as needed.
- Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility.
- Maintain accurate and timely clinical documentation.
- Collaborate with physicians, caregivers, and other healthcare professionals to coordinate patient care.
Qualifications
- Active New York State Physical Therapist license.
- Degree from an accredited Physical Therapy program.
- Strong clinical assessment and rehabilitation skills.
- Excellent communication and organizational skills.
- Previous physical therapy experience preferred.
- Ability to work independently and collaboratively.
