Job Title: IRP-Social Worker (HS)

Job Description Social work services include psyschosocial assessments, mental health counseling, crisis intervention, financial counseling, discharge planning, health education and the provision of material resources and linkage with community agencies. Social work services are provided to patients/families to meet their medically related social and emotional needs as they impact on their medical condition, treatment, recovery and safe transition from one care environment to another. Core competencies include : Psychosocial assessment, counseling and crisis intervention skills, Knowledge of the PASAAR process for extended care facility placement and the assessment and counseling processes involved, knowledge of home healthcare set-up process and skills assessing neglect/abuse, including substance abuse and domestic violence. Minimum Education Required Master's Level Degree Required Qualifications Master of Social Work from an accredited Graduate School of Social Work; State of Ohio Social Work licensure (LSW or LISW) or equivalent education/experience.
