Are you an experienced Dialysis RN looking for clinical autonomy, direct patient impact, and a predictable daytime schedule without the chaos of hospital on-call?
DaVita SNF Dialysis (DSD) is launching a brand-new on-site dialysis den suite at Foundations Saybrook Landing in Ashtabula, OH! In this specialized role, you will lead comprehensive renal replacement therapies inside a dedicated, modern skilled nursing dialysis suite.
Work Location: 2300 Center Rd, Ashtabula, OH 44004
- Set 3-day work weeks (~12-hour shifts) with NO overnight on-call and NO Sunday treatments .
Retention Bonus Opportunity This role is eligible for a retention bonus for dialysis-experienced candidates not currently employed by DaVita, paid out in milestone installments over your service with the Village. (Full terms, eligibility, and payout milestones will be detailed during the offer process.)
About DaVita SNF Dialysis (DSD) Approximately 10% of all dialysis patients reside in a skilled nursing facility (SNF) and are transported to outpatient clinics three times a week—placing a significant physical burden on patients and adding avoidable costs to the healthcare system.
DaVita SNF Dialysis (DSD) brings high-quality renal care directly on-site to residents inside an intimate, modern 6-chair dialysis den. In this specialized role, you serve as the clinical leader—partnering directly with a dedicated Patient Care Technician (PCT) to deliver life-sustaining hemodialysis care without the chaos of hospital on-call or crowded chronic clinics, backed by all the capabilities, technology, and resources DaVita has to offer.
What You Can Expect & What You’ll Do In this role, you take clinical leadership of comprehensive ongoing patient care, ensuring each resident's dialysis treatment stays safe, comfortable, and on track.
- Autonomous Clinical Leadership: Lead treatment operations within an intimate 6-chair den setting. Perform comprehensive pre- and post-dialysis assessments, evaluate fluid removal orders, monitor hemodynamic stability and vital signs, and manage complex vascular access (AV fistulas, grafts, and central venous catheters / CVCs).
- Direct Teamwork & Mentorship: Direct and collaborate closely with your dedicated PCT teammate, ensuring safe patient workflows, strict infection control, sterile protocols, and continuous quality care.
- Technical & Machine Operations: Operate, set up, and troubleshoot Fresenius hemodialysis machines (2008T / Bibag). Maintain oversight across water treatment systems—including both central water treatment rooms and individual portable Reverse Osmosis (RO) units—and ensure daily water quality testing and infection control standards.
- Holistic Interdisciplinary Care: Partner directly with SNF facility care teams, attending physicians, nephrologists, dietitians, and your Regional Operations Manager (ROM) to optimize clinical outcomes.
